Is the material used to make dental implant safe?
A dental implant is essentially a titanium metal screw, normally between 8 to 16 mm long, which is inserted into a prepared bony socket in the jaw. It will act as a replacement root for the missing tooth. A specially made titanium metal attachment called the abutment will be attached onto the implant, which forms the external connection for the new replacement tooth (crown) or teeth (bridge or denture).

During healing, the titanium surface of the implant fuses with the surrounding bone, in a process known as osseointegration, which can take about 3-6 months. After this time, the implant is sufficiently strong to support one or more false teeth.

When is dental implant treatment not possible?
It may not be possible to place implant if the jaw bone is not sufficiently thick to fit the dental implant and bone grafting is not possible. Also, implants are not suitable for patients with untreated gum disease or uncontrolled diabetes, because of the risk of failure through infection or poor healing.

There are other medical conditions that may contra-indicate the use of implants include blood clotting disorders; certain bone diseases, deficiencies in the immune system, severe systemic illnesses and drug abuse. Although smoking is not contra-indicated for implant, however smokers must be forewarned of lower implantation success rate because smokers heal less efficiently than non-smokers and are more likely to experience loosening of their implants. Quitting smoking will significantly improve your chances of successful dental implant treatment.

What does implant treatment involve?

  • Clinical Examination 
  • Your dentist will conduct a thorough examination of your mouth and will take x-ray of your jaws to assess the shape and condition of your bone tissue and identify the locations of any important structures, such as nerves and sinuses, which may be close to the intended implantation site. This will allow the implant dentist to plan exactly how the implants will be placed. In a more complex situation, a computed tomography (CT) scan may be used to provide three-dimensional images, but this is a more costly procedure and exposes you to more radiation than standard radiographs. Thus, it will be only recommended when necessary.
  • Placement of the dental implant
  • Implant surgery can be carried out using either a one-stage or two-stage procedure, depending on your particular clinical circumstances. In either case, the surgery involves making a small incision in the gum to expose the bone and using a special drill to shape the implant site. 
  • In the one-stage procedure, the implant is fitted into the prepared bone and then an attachment called a healing cap, which protrudes through the gum, is placed over it. This healing cap is removed after a 3-6 month period and replaced with a permanent attachment called the abutment, which takes the form of a post and core. The replacement tooth, or crown, is cemented in place over the abutment.
  • In the two-stage procedure, a cover screw is placed flush over the top of the implant and the gum is sewn up over it for the period of healing. A second procedure is needed to uncover the implant via a small incision in the gum and attach the healing cap. Then, after the gums have healed, the healing cap is removed and replaced by the abutment, in readiness for the new crown.
    Your implant dentist will use his/her clinical judgment and experience to select the appropriate procedure for your implant therapy.

  • When your dental implant has fully healed (usually after healing period of 3 months), your dentist will take a new mould of your implant. This will be used to make a model of your teeth on which the dental technician will fabricate your implant crown or bridge, trying to achieve a natural finish that mimic the rest of your natural dentition.
  • Installation of crowns or bridges
  • When your new crowns and bridges have been fabricated, your implant dentist will cement or screw them on to the abutments over the dental implants, assess the appearance and fitting, and check that your upper and lower teeth work together comfortably when you bite. You will usually need to return for several clinical review sessions to make sure that the implants are stable and functioning appropriately, and that you are completely comfortable and pain free with the new implant prosthesis.

What is bone grafting and bone regeneration?

Bone grafting procedure is performed to increase the width and height of the jawbone at the implant site. The best success can be achieved when bone from another part of your own mouth or body is used and this is known as autogenous grafting. Typically, bone is harvested from inside the mouth, as it is a good source of marrow which contains many bone cells. Allograft is a graft that use donated human bone taken from a 'bone bank' and work in the same way as autogenous grafts but with lesser success rate. Xenograft is a procedure that use bone from an animal donor, with cows acting as the main source (bovine bone). The bovine bone in the graft is replaced by your own bone tissue over time. Alloplast is grafting that use synthetic bone substitutes that are chemically similar to human bone. These grafts act as a sort of framework for new natural bone formation and may be replaced by natural bone (resorbable alloplast) or retained (non-resorbable alloplast).

An autogenous graft is the preferred choice because there is absolutely no risk of your body rejecting it. After the procedure, you will need to allow appropriate healing for at least 3-6 months before having implants placed. The exact waiting time will depend how much bone has been grafted.

This is a special surgical method to enable bone cells an advantage over other types of cell to multiply and cover an area where bone loss has happened. Frequently, cells from the gum surface and the connective tissues multiply much faster at filling the gaps created by bone destruction compared with the bone cells and so bone may not have an opportunity to regenerate even when inflammation has been eliminated. In this procedure, the barrier membrane covers the area of bone loss and prevents access to it for the other cell types, giving the bone cells a better opportunity to fill in the defect. The barrier membrane may disintegrate and disappear over time (resorbable) or may need to be removed in a later procedure (non-resorbable).

1. Do I need to take out my dental bridge?

No. Dental bridge is a fixed dental replacement option. It is permanently cemented in place and remains all day and all night.

2. Can I eat normally with dental bridge?

Yes. The dental bridge will function as your normal teeth would. In other words, you don’t have to worry about chewing and eating with the dental bridge. However, hard foods like nuts can damage the structure of dental bridge, just like eating hard food with your normal teeth will increase the risk of teeth fracture as well.

3. How long will the dental bridge last?

Generally, bridges can last anywhere between 5 and 15 years or even more. The key is proper oral and dental care. Routine teeth cleaning and check-up are required to increase the lifespan of dental bridges.

4. How do I take care of my dental bridge?

Food particles can slip underneath the bridge, hence floss is needed to adapt underneath the bridge to keep that area clean.  Routine check-up every 6 months is required to detect early failure of dental bridge.

5. Can I get my dental bridge in one visit?

No. During the first visit, consultation and X-rays will be done to determine whether your case is suitable for dental bridges. After a treatment plan is made, the process of getting a bridge takes two visits.  In the first visit, the teeth are shaped and an impression is taken and sent to the lab.  A temporary bridge is made and cemented.  At the second visit, the temporary bridge is removed and the permanent bridge is cemented in.

6. Are there different types of dental bridges available?

Yes. There are fixed-fixed dental bridges, Cantilever bridges, Maryland bridges, and implant-supported bridges. Fixed-fixed dental bridges are the ones involving crowns on either side of the gap. A Cantilever bridge is similar, except it only needs the support of a crown on one side. A Maryland bridge doesn’t need a dental crown for support because it’s held in place by a framework that’s bonded on the back of the adjacent teeth without needing to trim them down. An implant-supported bridge uses dental implants for support instead of adjacent teeth. Do consult your dentist to determine which type of bridge works best for your needs.

Removable Dentures

1. Do I need to remove my dentures at night?

Yes. You should remove your dentures at night and this will allow give your gum tissue to rest from the pressure of the denture and allows normal stimulation and cleansing by the tongue and saliva. Dentures should be cleaned at night and stored in water during the night.

2. Can I eat normally with my denture?

You need to learn how to use dentures properly and hence it takes a little time to get used to them. After a while, you should be able to eat fairly normally. Start out by eating soft foods that are cut into small pieces. Chew on both sides of the mouth to keep even pressure on the denture. Avoid foods that are extremely sticky or hard. You may want to avoid chewing gum.

3. Will the denture change the way I speak?

You may find it difficult to pronounce certain words with your new denture. Practice reading out loud and repeat the words that give you trouble. With time, you will become accustomed to speaking properly with your denture.

4. Can I get my denture in one visit?

No. It takes a few weeks and normally 3-5 appointments. These include: Impressions – a mold is taken of your gums and/or teeth; Bite registration – we measure how you bite and select your shade of teeth; Wax try-in – you get try the denture on before they are actually finished; Finish – you get to wear your new denture home.

5. How do I take care of my dentures?

Dentures are porous and prone to absorbing stains over time. Scrub them with a soft toothbrush using soap and water. Toothpaste can be abrasive and will wear down the gloss coating and make them prone to staining. 

6. How long can I start to do my denture after teeth extraction?

It takes about 4-8 weeks for the gum to heal properly after extraction of the teeth. Hence, it is advisable to start fabricate the denture after 4-8 weeks to ensure a good fit for the denture.

🦷Korg perlu pakai retainer ni selama 24/7 (masa tidur pun kena pakai!) utk setahun pertama selepas braces dibuka, tapi masa makan/minum perlu buka, termasuklah minum kopi, teh atau air berkarbonat pun, cuma air kosong je yg boleh diminum sambil pakai retainer ye.
Lepas setahun, bergantung pada nasihat dr, korg hanya perlu pakai retainer di waktu malam (masa tidur) je, bermula dari tahun kedua & seterusnya😌

7) Macam mana cara penjagaan retainer ni ye?
🦷Kami akan sediakan 'kotak retainer' utk korg simpan retainer tu setiap kali dibuka.
Retainer lutsinar (Essix retainer) ni mmg agak mudah patah kalau tak dijaga baik2. Tak susah pun nak jaga sebenarnya , asalkan setiap kali buka, pt perlu simpan dalam kotak retainer korg.
JANGAN sesekali bungkus dgn tisu atau letak atas meja/kerusi je.
Dah ramai pt kami yg terpaksa buat retainer baru sebab retainer lama tu hilang/patah, ni cuma akan terjadi kalau tak simpan retainer dlm kotak retainer yg dah disediakan😬
