🤔 Apa fungsi sebenar pendakap gigi (braces) ?

💡Braces ialah Antara cara paling berkesan utk meluruskan susunan gigi kita. Gigi yg tersusun memastikan proses membersihkan gigi jadi lebih mudah. Penampilan kita pun akan nampak lebih bagus.

🤔 Macam mana cara braces berfungsi?

💡Braces berfungsi dgn cara menggerakkan gigi menggunakan wayar & brackets besi yg akan di servis setiap bulan. Jadi korg kena rajin dtg setiap bulan ye!

🤔 Wajib ke cabut gigi kalau nak pakai braces?

💡Tak semestinya. Dr akan cek gigi korg, bergantung pada masalah gigi, result yg korg nak dapatkan & xray gigi, dr akan bgtau samada kes korg perlukan cabutan atau tak.

🤔 Sakit ke bila pakai braces ni?

💡Pada seminggu pertama, mungkin akan rasa tekanan sikit yg disebabkan pergerakan pada gigi, tapi lepas tu korg akan mula biasa & tak rasa sakit dah.

🤔 Berapa lama saya perlu pakai braces?

💡Bergantung pada keadaan gigi setiap individu. Masalah gigi yg berbeza, result yg diingini setiap patient, semua ni memainkan peranan dlm menentukan tempoh masa pemakaian braces korg.
Masalah gigi yg lebih teruk, memerlukan masa yg lebih panjang utk siap.
Tapi kebiasaannya akan ambil 1-3 tahun utk selesai.

🤔 Umur berapa yg paling sesuai utk braces?

💡Remaja 17-23 tahun adalah paling sesuai utk rawatan braces sebab tulang lebih lembut, jadi pergerakan gigi lebih cepat.
Tapi takde masalah utk lingkaran umur lain nak pakai braces. Yg penting semua gigi kekal dah keluar & gigi cukup kuat serta stabil.

🤔 Apa itu retainer?

💡Retainer ialah alat yg akan memastikan gigi kita kekal pada kedudukannya selepas braces dibuka.
Utk setahun pertama, retainer perlu dipakai 24/7. Lepas tu cuma perlu pakai malam masa tidur je.
Retainer akan memastikan fiber & ligamen pemegang gigi di kedudukan baru tu terbentuk dengan sempurna & kekal kat situ.

🦷Cabutan gigi sebelum pemasangan braces ialah biasa & dilakukan atas beberapa faktor.
Salah satu sebab yg paling biasa ialah kedudukan gigi yg terlalu padat/rapat, jadi 1 atau lebih gigi perlu dicabut utk beri ruang supaya gigi lain boleh bergerak ke kedudukan yg sepatutnya.
Jika gigi dah tumbuh sepenuhnya, ia boleh dicabut dgn cabutan biasa, tapi utk gigi yg rosak atau terimpak (terbenam dlm gusi), cabutan secara pembedahan kecil perlu dilakukan sebelum pemasangan braces😗

4) Berapa lama saya perlu pakai Transparent braces?
🦷Biasanya rawatan braces akan ambil masa 1-2 tahun utk selesai, bergantung pada keadaan gigi.
Berdasarkan pengalaman dr kami, biasanya braces akan ambil masa 1 1/2 tahun utk selesai🤔

5) Apa yg perlu saya lakukan selepas tamat rawatan Transparent braces?
🦷Selepas rawatan braces selesai, Brackets (braces) akan dibuka.
Gigi akan di'polish' & dibersihkan, selalunya rawatan cuci gigi akan dibuat sekali.
Selepas tu, dr akan mulakan proses utk membuat retainer.
Acuan gigi atas & bawah akan diambil, lepas beberapa hari, retainer korg akan siap! 👏🏻

6) Berapa lama retainer ni perlu dipakai?
🦷Korg perlu pakai retainer ni selama 24/7 (masa tidur pun kena pakai!) utk setahun pertama selepas braces dibuka, tapi masa makan/minum perlu buka, termasuklah minum kopi, teh atau air berkarbonat pun, cuma air kosong je yg boleh diminum sambil pakai retainer ye.
Lepas setahun, bergantung pada nasihat dr, korg hanya perlu pakai retainer di waktu malam (masa tidur) je, bermula dari tahun kedua & seterusnya😌

7) Macam mana cara penjagaan retainer ni ye?
🦷Kami akan sediakan 'kotak retainer' utk korg simpan retainer tu setiap kali dibuka.
Retainer lutsinar (Essix retainer) ni mmg agak mudah patah kalau tak dijaga baik2. Tak susah pun nak jaga sebenarnya , asalkan setiap kali buka, pt perlu simpan dalam kotak retainer korg.
JANGAN sesekali bungkus dgn tisu atau letak atas meja/kerusi je.

❤️ Menurut 'National Oral Health Survey Of Adults 2010', kajian menunjukkan yg kurang dari satu pertiga dari org dewasa menggunakan servis pergigian dalam setahun, sedangkan kita perlu dapatkan pemeriksaan gigi secara tahunan.


Ramai yg rasa mcm tak perlu jumpa dr gigi pun kalau takde rasa sakit atau gigi rosak.

Hakikatnya, simptom awal penyakit gusi/kerosakan gigi mmg takkan sebabkan rasa sakit, bila dah mula rasa sakit tu, maksudnya korg dah pun terlambat.

🧡 Ini sebabnya :

*Karies gigi.


Bila makanan yg tersekat dicelah gigi kita tu tak betul2 dibersihkan, bakteria dlm mulut kita akan 'makan' sisa makanan tu & menghasilkan asid.
Asid ni lama2 akan 'demineralize' permukaan gigi kita, menyebabkan kerosakan.

*Kalau gigi dah mula rosak, kenapa tak rasa sakit?


Lapisan luar gigi kita terdapat enamel. Enamel ni terdiri daripada mineral yg sgt kuat, digelar 'calcium phosphate'.
Bila asid dari bakteria menyerang, enamel akan jadi pertahanan depan yg akan melindungi gigi.
Kalau kerosakan gigi dpt dijumpai semasa diperingkat awal, boleh rawat dgn tampalan biasa je lagi.

Kalau tak, kerosakan tu akan jadi makin dalam, sampai ke lapisan yg kita panggil 'dentin'.
Di lapisan dentin ni, terdapat satu sel yg berfungi utk menghasilkan mineral.

Dentin lebih lembut berbanding enamel, jadi kerosakan bahagian ni lebih cepat merebak & jadi makin teruk.

Malangnya, kalau bakteria dah smpi ke lapisan paling dalam, yg kita panggil 'pulp tissue', gigi tu dah tak boleh dirawat dgn tampalan biasa lagi dah.
Jadi dr akan sarankan korg utk samada cabut je gigi tu, atau selamatkan dgn cara rawatan akar (RCT). Kalau tak, korg akan alami rasa sakit teramat pada gigi tu.

*Penyakit gusi


Nafas korg berbau tak? Masa gosok gigi, ada berdarah ke?
Ni benda2 biasa yg selalu menganggu kita kan. Apa penyebab masalah ni terjadi?

Ni semua di akibatkan oleh plak gigi!


Plak ialah lekitan warna putih yg berkumpul pada permukaan gigi, disebabkan oleh bakteria. Lebih2 lagi kalau bnyk makan makanan manis, lebih mudah plak terbentuk.

How often do you see a dentist in a year?

According to the National Oral Health Survey of Adults 2010, the study indicated that only less than one-third of adults utilised oral healthcare services in the past year when almost everyone needed dental services annually. Many people feel that they do not need to visit a dentist when they do not feel any pain or discomfort in the oral cavity.

In fact, the early signs of disease in the mouth do not cause pain or discomfort. When pain occurs, usually it is too late. Prevention is better than cure and too prevention stage is more affordable than having to cure. 

Here’s why:

Dental caries/decay

When the food that we eat was not cleaned properly and sticking onto our teeth, bacteria in the mouth happily “eats” the food that was left and produces acids. These acids overtime will demineralise our tooth surface, causing decay. 

Stages of tooth decay

Why pain was not felt when there is decay? 

The most outer layer of our teeth is consisting of enamel. Enamel is made up of mostly of an extremely hard mineral called calcium phosphate. When the bacteria acid attacks, enamel will be the “front liner” to protect the tooth. If decay was found at early stage, prevention, remineralisation or simple fillings still can take place. 

Otherwise, decay will progress deeper, into the next layer called dentin. In dentin layer, it consists of living cells that secrete mineral substances. The living cells in dentin have nerve endings that will cause pain and sensitivity when they are injured, this is why you start to feel some discomfort. Dentin is comparatively softer than enamel, decay will progress faster when bacteria went in deeper. 

Unfortunately, if bacteria go into deepest layer, which is the pulp tissues (which consists of nerve and blood vessels), the tooth is no longer can be filled. You will eventually be advised by dentists to remove the tooth or perform root canal treatment in order to relieve pain. 

Gum disease

Do you have bad breath? Do you bleed when you brush? 

These questions are common and have been troubling our lives from day to day. So, what are the causes to these problems? 

Dental plaque is the culprit! 

Plaque is a sticky film of bacteria that constantly forms on teeth few hours after food. Plaque forms when bacteria in your mouth mix with sugary or starchy foods. These acids can destroy tooth enamel and cause cavities and gum disease. Plaque gets hardened and form stones like structures (dental calculus) in your gums. Then gums get swollen, inflamed, starts to bleed and smell bad. 

Therefore, scaling was carried out to remove plaque and dental calculus beneath the gums. If treatment is delayed, the gum situation worsens (progresses to periodontitis) and teeth might become shaky as the bacteria has eaten up the surrounding bones causes the teeth to lose support. Eventually, more complex treatment like root planning/ surgery/ tooth removal will be needed. 

Stages of gum diseases

In conclusion, if dental visits were done regularly (recommended twice a year), decay and gum diseases were detected at early stage, simple filling and scaling can be done at a lower range of fees. Complex treatments such as root canal treatments, root planning, tooth extractions and replacement afterwards can be avoided. 

General check-up twice a year along with full mouth diagnostic x-rays (recommended to be taken once in two years) are essential to maintain a healthy lifestyle with brighter smile.

Apa tu gigi bongsu?

💡Kebanyakan orang bila dah masuk usia remaja, akan ada satu gigi yg akan tumbuh di hujung sekali, tu yg kita panggil gigi bongsu!

Gigi bongsu ialah gigi yg tumbuh paling belakang sekali & selalunya ada 4 unit gigi ni (atas, bawah, kiri, kanan). Biasanya gigi ni akan tumbuh pada umur 17-21 tahun.
Tapi takde kegunaan yg spesifik utk gigi ni, berkemungkinan utk membantu mengunyah makanan.

Masalah dental yg berkaitan dgn gigi bongsu ni boleh terjadi disebabkan ia lambat tumbuh & kekurangan ruang utk tumbuh sepenuhnya.
Gaya hidup moden sekarang pun memainkan peranan dlm pertumbuhan gigi kita, sebab bila kita makan bnyk makanan proses yg lembut, ia menjadikan pertumbuhan rahang kita terbantut.

Kalau gigi bongsu dah tumbuh sepenuhnya tanpa apa2 simptom, gigi tu tak perlu dicabut.
Cuma kalau dah ada simptom yg mengganggu hidup seharian patient, jadi lebih baik utk gigi ni dicabut.

Apa akan terjadi kalau gigi bongsu tu tak cukup ruang utk tumbuh?

💡Ada beberapa perkara yg mungkin akan terjadi kalau gigi bongsu takde ruang yg cukup utk tumbuh, antaranya :

  • kerosakan gigi - impak dari gigi bongsu dgn gigi didepannya (gigi geraham kedua), akan menyebabkan makanan tersekat dicelah antara kedua-dua gigi.
    Tmpt ni susah nak dibersihkan & menyebabkan dua2 gigi akan mula rosak.
    *bengkak/bernanah - sesetengah gigi bongsu tak dpt tumbuh sepenuhnya. Kadang2 tu tumbuh separuh je. Jadi gusi disekeliling gigi ni akan jadi tmpt pengumpulan bakteria, yg akhirnya menyebabkan sakit, bengkak & susah utk buka mulut.
    *ketumbuhan - gigi bongsu ni boleh menyebabkan ketumbuhan jugak tau. Kalau dah smpi ke tahap ni, mmg dah perlu cabut ye.

Proses rawatan utk gigi bongsu

💡Memang dinasihatkan utk gigi bongsu ni dicabut, terutamanya kalau dah ada simptom yg smpi menjejaskan kehidupan patient.
Cabutan gigi bongsu ni memerlukan pembedahan kecil.

Pertama sekali, sesi konsultasi dgn dr diperlukan. Dr akan tanya tentang simptom & latar belakang masalah kesihatan yg lain sebelum memulakan langkah seterusnya.
Lepas tu, dr akan menjalankan pemeriksaan gigi utk tgk jika ada kerosakkan pada gigi tu & sekitar gusi. X-ray pun perlu ambil, untuk pemeriksaan secara mendalam sekaligus untuk merancang rawatan yg diperlukan untuk keadaan gigi bongsu tu.

Selepas dr berbincang tentang plan rawatan, patient boleh teruskan dgn prosedur lepas dah baca & menandatangani borang kebenaran.
Prosedur ni boleh dijalankan samada dgn menggunakan 'local anesthesia' di mana kawasan pembedahan akan dikebaskan & patient akan berada dlm keadaan sedar sepenuhnya.

Selepas gigi bongsu dah berjaya dicabut, dr akan letakkan beberapa jahitan dibahagian surgeri & pt akan disuruh utk gigit kapas utk hentikan pendarahan.

Dr akan sediakan ubat tahan sakit & antibiotik utk pt.
Lepas seminggu, pt perlu dtg semula utk periksa bahagian surgeri tu & buka jahitan.

Penjagaan selepas prosedur cabutan gigi bongsu (Post-Operative Care)

💡Ada beberapa perkara yg perlu pt lakukan selepas surgeri :

  • gigit kapas smpi darah berhenti
    *makan makanan lembut & sejuk/suam. Elakkan makanan yg keras, panas & pedas.
    *elakkan berkumur terlalu kerap & pastikan kawasan surgeri kekal bersih
    *rehat secukupnya
    *makan ubat mengikut arahan dr
    *berhenti merokok sekurangnya 3 hari

Komplikasi yg mungkin terjadi

💡Ada beberapa komplikasi yg mungkin akan terjadi dari surgeri ni, antaranya :

*'dry socket' - kekurangan darah beku lepas cabutan akan buat tulang bahagian surgeri tu terdedah & menyebabkan rasa sakit
*jangkitan - jangkitan boleh terjadi bila bahagian surgeri di serang bakteria atau ada sisa makanan yg tersangkut
*kerosakan saraf (nerve damage) - rasa kebas akan terjadi kalau ada kerosakan saraf semasa prosedur surgeri, samada sementara atau kekal.

🌸Patient disarankan untuk mendapatkan konsultasi dari dr utk tahu lebih lanjut tentang keadaan gigi bongsu masing2 & apa langkah seterusnya yg perlu diambil.

What is Wisdom Tooth and What Is Treatment Process for Wisdom Teeth

Wisdom Teeth

Background:

Most people upon reaching late adolescent will experience their last teeth growing out from their gum. This tooth is known as the wisdom tooth.

Wisdom teeth are each of the four hindmost molars in the oral cavity. These teeth typically erupt between 17 years old to 21 years old.  There are no known specific purpose for such teeth, perhaps it exist to help grind food. Dental problems in regards of such teeth could occur due to the late eruption and lack of space for such teeth to erupt well into the dental arch. Modern diet also plays a role in the development of our wisdom teeth. In the modern age we tend to consume a lot of processed foods which are soft that will stifle the growth development of our jaw.

If the wisdom teeth fully erupt into the dental arch without any symptoms then such teeth arenot needed to be removed. Removal of offending wisdom teeth are considered when there are signs symptoms present which in turn will affect the patient’s quality of life 

What would happen if the wisdom teeth have no space to grow?

A number of things may happen if the wisdom teeth have no space to grow.

  1. Tooth decay: Impaction of wisdom tooth and the tooth in front of it may cause food to get stuck in between. This area is difficult to clean and after a while, tooth decay may develop between both teeth.
  2. Swelling/Abscess: Some wisdom teeth may not erupt fully and abit of the tooth could be seen. The gum around the teeth can be a reservoir of bacteria accumulation as food debris easily lodges into the gum. Pain, swelling and inability to open mouth can happen at this stage.
  3.  Cyst/tumours: Cyst and tumours can grow out of the wisdom teeth. At this stage, removal of the offending tooth will be recommended.

Treatment process for Wisdom Tooth

It is advisable to have the wisdom teeth removed especially when there are signs and symptoms that the might affect the patient’s quality of life. Removal of the wisdom teeth usually involves a minor oral surgery. 

Firstly a consultation with a dentist is needed. Whereby the dentist will ask the patient questions regarding the symptoms and thorough medical history will be recorded before proceeding to the next step. The dentist will also conduct a dental examination of the oral cavity where the dentist will check on the offending tooth and areas around it. X ray may it be a periapical or OPG will be taken to further diagnose and making a treatment plan for the case.

Once the dentist has presented the treatment plan to the patient, the patient may proceed with the procedure after reading and signing the consent form. The procedure can be done under local anesthesia where the surgical area will be numbed and the patient will be fully awake. This procedure is done by reflecting a surgical flap around the tooth and removal of some bone. The tooth may be removed as a whole piece or sectioned into pieces for easier removal.

Once the tooth is removed, stiches will be placed along the operated area and the patient will be asked to bite on a gauze to stop the bleeding. The dentist will provide medications such antibiotics and pain killers for the patient to bring home. 

After about week after the surgery, the patient is required to return to the clinic for a review. The dentist will perform an examination on the wound and remove the stitches.

Post-operative care:

There are a few things to take note after the surgery:

  1. Bite on the gauze until the bleeding stops.
  2. Consume soft warm food/drinks. Avoid hot and spicy food/drinks.
  3. Avoid gargling and keep the surgical area clean.
  4. Have enough rest.
  5. Take medication as prescribed.
  6. Avoid smoking

Complication that may arise:

Some complications may arise from the surgical removal of the wisdom teeth

  1. Dry socket: Absence of blood clot in the extraction socket leaving the bone exposed causing pain.
  2. Infection: Infection can occur if the surgical site is invaded by bacteria or food particles are trapped.
  3. Nerve damage: Numbness usually happens when nerve is damage during the procedure.

Conclusion:

It is advisable for the patient to get a consultation from a dentist to help make a better decision on how to manage their case. A routine dental check up every 6 months should also be done.

Root Canal Treatment : Your Tooth Deserve a Second Chance

                  According to National Oral Health Survey for Adults 2010, dental caries prevalence was found to be 90% in year 2000.

Dental caries or commonly known as tooth decay are disease conditions where the teeth are attacked by acid producing bacteria found in oral cavity, which causes loss of minerals (demineralization), resulting in weaker tooth structure which are prone fracture and decay. Decayed tooth starts with small cavity which can be detected during a routine dental check up every 6 monthly. Takk, ada bnyk lagi kelebihan utk rawatan braces ni.
Gigi yg tersusun rapi akan memudahkan kita utk jaga kebersihan gigi dgn lebih bagus.

Posisi gigi terbaru haruslah stabil. Bagaimanapun, korg haruslah memakai braces sepanjang tempoh yang dinasihati oleh doktor gigi korg.
Ini akan membantu memgelakkan gigi bergerak ke posisi asal semula sebelum pemakaian braces. Walaupun hanya sedikit pergerakan, tapi nanti rasa mcm sia2 la usaha & duit yg korg dah belanjakan utk rawatan braces tu kan.

Bracket tu halus jadi korg kena membersihkannya dengan cermat supaya ia tak asyik tercabut je.

🦷Dr korg anda akan ajar cara2 penjagaan mengikut jenis braces yang dipakai.

Selain itu, kurangkan makanan dan minuman yang manis. Juga elakkan makanan yang melekit & keras sebab ia akan merosakkan braces anda.

Akhirnya, berus gigi sebelum tidur pada waktu malam & sekali pada waktu siang. Gunakan ubat kumur mulut jika diperlukan.

Who usually has orthodontic treatment and wears braces?

The best time to have braces is generally during childhood. But adults can have orthodontic treatment too – and more and more are doing this. Age is less important than having healthy teeth and gums. Children may have to wait for enough teeth to come through before starting treatment.

It is important that your mouth is fit and healthy before you have orthodontic treatment. Some patients can’t have orthodontic treatment – for example, if they have advanced gum disease. Only an orthodontist can decide whether you are able to have orthodontics.

Why might I need braces?

There are many reasons why you might want, or be advised, to have braces.

Some of the most common reasons are:

overcrowded or crooked teeth
not enough, or too much, spacing between your teeth
upper front teeth that stick out (called an 'overjet')

Is orthodontic work permanent?

It’s always possible that teeth might move. Orthodontic treatment aims to move the teeth into a stable position. This is complicated and needs a specialist understanding of where teeth can be moved to, and how.

The new position of your teeth should be a stable one. However, you should wear your braces for as long as your orthodontist advises. This will help stop your teeth from moving back to where they were. Even if they do this only slightly, you may be disappointed with the result.

You'll also need to wear a ‘retainer’ regularly for some time after your treatment has finished. This stops your teeth moving back to the position they were in before the treatment. We don’t want all the hard work you’ve put in up to this point going to waste!

How do I care for my brace and teeth?

It is important to keep having your teeth checked by your dental team, at routine check-ups, while you are having orthodontic treatment and afterwards.

Clean your teeth carefully every day, including between your teeth where you can. Braces are delicate and you need to make sure you clean them carefully so that they do not break. Your dental team will be able to show you how to do this for the type of brace you are wearing.

Again, cut down on how much sugary food and drink you have and how often you have them. Also, sticky and hard foods may damage your brace.

Finally, brush your teeth last thing at night and at least one other time during the day. Use a mouthwash if you need to. Your dental team may recommend a stronger fluoride toothpaste, or perhaps a fluoride gel or mouth rinse, for you to use.

How successful are braces?

In most cases, orthodontic treatment works well, but you need to be committed to your treatment for it to be successful. Follow the instructions your orthodontist gives you.

While you are wearing braces, you need to be more careful about what you eat. To give your braces the best possible chance of improving your teeth, cutting back on sugary and acidic foods is a must. This is because braces can trap bits of food and cause more plaque to build up than usual. Plaque is a thin, sticky film of bacteria that builds up on your teeth.

You should keep seeing your regular dentist while having orthodontic treatment, and after your treatment has finished.

Is an X-Ray needed to check my wisdom teeth?

Yes, a full mouth (OPG) xray will be needed to check whether your wisdom tooth is close to your main nerve. There is risks involved in removing your wisdom tooth , which is if (Touch wood) when removing your wisdom tooth, your main nerve is accidentally touched , you will be feeling numbness around your chin and lips area

When is the best time to remove my wisdom tooth ?

The best time to schedule for a removal is when you have less work and able to rest for 3-4 days after the surgery. Normally our dentist will be giving your MC (medical certificates) to rest for at least 2 days .

Do I need to be Fully Asleep (Full Unconscious) during the surgery of my wisdom tooth ?

Normally removal of wisdom tooth need not be in the operating theatre and can be done in general dental clinic setting. Local Anaesthetic will be given during the surgery of your wisdom tooth and you wont feel a thing during removal. All you have to do is to stay calm and relax during the procedure and our dentist will most probably be able to remove your wisdom tooth smoothly .

Do I need to remove my wisdom tooth before braces treatment ?
